The Failure Mode and Effect Analysis (FMEA) has proven effective in preventing failures in manufacturing companies. What is missing from this tool are functions to financially evaluate the failures and continuously review the inspection plans. To fill this gap, the Fraunhofer-Institute for Manufacturing Engineering and Automation (IPA) developed the Failure Process Matrix (FPM).
