Biologics in asthma management - Are we out of breath yet?
Abstract
Die Biologika, die für die Add-on-Therapie des schweren Asthmas zugelassen sind, gehören zu den monoklonalen Antikörpern. Bevor sie für eine Therapieintensivierung in Frage kommen, wird anhand phänotypischer Merkmale auf den Asthma-Endotyp des Patienten geschlossen. Für die Signalwege des ""TH2-high""-Endotyps, bei denen Mediatoren der von Typ-2-Helfer-Lymphozyten vermittelten Entzündungskaskade hochreguliert sind, stehen bislang fünf Präparate zur Verfügung. Der korrespondierende Phänotyp dieses Entzündungs-Endotyps ist das schwere eosinophile Asthma mit Erhöhung eosinophiler Granulozyten, Immunglobulin E und fraktioniertem exhaliertem Stickstoffmonoxid (FeNO). Im Gegensatz dazu ist der heterogene ""TH2-low""-Endotyp bislang noch nicht ausreichend verstanden. Häufig beschrieben ist bei dieser Variante eine Erhöhung von neutrophilen Granulozyten im Sputum sowie eine vermehrte Expression des TH17-Lymphozyten-vermittelten Interleukin-17-Signalwegs. In der klinischen Prüfung befinden sich zahlreiche Biologika, von denen besonders die Antikörper gegen Thymic Stromal Lymphopoietin (TSLP) vielversprechende Ergebnisse unabhängig vom Phänotyp gezeigt haben.

The biologics authorized for the add-on therapy of severe asthma are monoclonal antibodies (mAbs). Before they are considered for therapy intensification, the patient's asthma endotype is determined on the basis of phenotypic characteristics. So far, 5 biologics are available that target the signaling pathways of the "TH2-high" asthma endotype, in which cytokines of the inflammation cascade mediated by type 2 T-helper cells are upregulated. The corresponding phenotype of this inflammatory endotype is severe eosinophilic asthma, with elevated eosinophils, immunoglobulin E, and fractional exhaled nitric oxide (FeNO). In contrast, the heterogeneous "TH2-low" endotype is not yet sufficiently understood. Frequently described in this variant is an increase of sputum neutrophils and an increased expression of the TH17-mediated interleukin-17 signaling pathway. There are numerous biologics currently in clinical trials, the thymic stromal lymphopoietin (TSLP) mAbs in particular have shown promising results independent of the asthma phenotype.
