Artificial Cornea for ultima ratio patients
Abstract
Korneale Erblindungen aufgrund immunologischer Oberflächenerkrankungen des Auges wie dem vernarbenden Pemphigoid oder dem Lyell-Syndrom können nicht erfolgreich durch perforierende Keratoplastiken therapiert werden. Für diese Patienten kommen im Endstadium als Hornhautersatz ausschließlich künstliche Hornhäute, sogenannte Keratoprothesen, infrage. Es wird eine neue vollsynthetische Keratoprothese vorgestellt, die zur Implantation bei ultima-ratio Patienten vorgesehen ist. Die Verankerung der künstlichen Hornhaut mit dem umliegenden Gewebe wird durch eine zellfreundliche Beschichtung der Haptikoberfläche der Prothese erreicht. Die Optik dagegen wird durch eine andere Beschichtung transparent gehalten. Die optischen Eigenschaften des zentralen Zylinders können auf die individuelle Brechkraft des Empfängers hin abgestimmt werden. Gegenwärtig ist die gezielte Anwendung dieser Prothese nur als letzte Hilfe für Patienten indiziert, bei denen herkömmliche Therapieansätze unzureichende Aussichten auf Erlangen der Sehkraft versprechen.

Penetrating keratoplasty is not suitable for the treatment of corneal blindness caused by immunopathologies of the surface of the eye, such as scarring pemphigoid or the Lyell-syndrome. Instead, fully synthetic corneal replacements, referred to as keratoprostheses, need to be considered for therapeutic approaches. Here, we introduce a fully synthetic keratoprosthesis, implanted into eyes of patients with severe corneal pathologies. Firm anchorage of the keratoprosthesis was achieved by a coat of the rim promoting growth of cells and deposition of extracellular matrix. A different coat at the center, the region of the optic, ensured transparency. The optical properties of this central cylinder can be tailored to the patients needs. The intended use of this keratoprosthesis is reserved for the treatment of cases where conventional therapeutic approaches will fail to restore eye sight.
