Chirurgiesystem für die roboterassistierte transpedikuläre Verschraubung
Abstract
Aufgrund einer zunehmenden Zahl von Patienten mit Rückenproblemen müssen Eingriffe an der Wirbelsäule immer häufiger vorgenommen werden. Als Standardverfahren zur operativen Behandlung gilt in Europa die manuelle Versteifung der Wirbelsäule mit Hilfe der transpedikulären Knochenverschraubung. Für diesen Eingriff werden zwei Löcher in den Wirbelkörper gebohrt und Schrauben von Hand eingedreht, wobei die akute Gefahr der Verletzung der Nerven im Spinalkanal der Wirbelsäule besteht. Deshalb sind hier eine besonders hohe Präzision und Reproduzierbarkeit gefordert. Bei Eingriffen im Hals- und Brustwirbelbereich stößt die Bewegungsgenauigkeit des Operateurs sogar an ihre Grenzen. Zur Unterstützung des Chirurgen entwickeln die Fraunhofer-Institute IPA, IGD, IPK und IBMT im Rahmen eines FuE-Projektes "Roboter und Manipulatoren für Medizinische Einsatzfelder - RoMed" ein Robotersystem zum Einsatz in der Wirbelsäulenchirurgie. Aufbauend auf den Erfahrungen bei der Konzeption eines Prototypen zur Anwendung in der endoskopischen Neurochirurgie am Fraunhofer IPA liegt der Projektschwerpunkt neben der Optimierung des Roboterbasissystems auf der Weiterentwicklung der benötigten Peripheriesysteme, z.B. Navigationshilfen, Eingabegeräte und Instrumente. Mit dem Ziel der Integration zu einem funktionsfähigen System wurden neue innovative Lösungen für die peripheren Komponenten gefunden.

Due to the increasing number of people suffering under back problems, spine surgery depicts an ever more frequently tasked therapy. The manual fusion of the spine with transpedicular screw fixation is deemed to be the standard technique for operational handling in Europe. This means two holes are drilled in the vertebra and the screws are twisted manually, which holds the risk to injure the nerves in the spinal column. Therefore high precision and reproducibility are required. With the vertebra thoracica and cervicalis the movement accuracy of the operating surgeon is limited. In order to support the surgeon the Fraunhofer-institutes IPA, IGD, IPK and IBMT are developing a robot system for use in spine surgery within the project "Robots and manipulators for the use in medical fields -RoMed". With the experiences of the Fraunhofer IPA from the conception of a prototype for the application in endoscopic neurosurgery the emphasis of this project is the advancement of the necessary peripheral systems like support aid systems, input devices and actuators. With the target of the integration to a functional system new innovative solutions for the peripheral components were found.
