Abstract
Acute Myeloid Leukaemia (AML) is a heterogeneous group of malignancies of the haematopoietic system. Its subtypes M4 and M5 are characterised by an enhanced expression of CD64 (Fc gamma receptor I), the high affinity receptor for IgG. Whereas conventional therapies such as chemo- and radiotherapy unspecifically affect cells with a high proliferation rate, and thus elicit undesired side effects due to the elimination of healthy cells, immunotoxins can specifically bind and kill malignant cells. The utilisation of human protein components, e.g. the human serine protease Granzyme B as a toxin, contributes to the absence of side effects and hence safety of an immunotoxin. In this thesis, the first humanised Granzyme B-based immunotoxin directed against CD64-positive cells was developed, evaluated and modified. Prior to these studies, the sensitivity of the AML-related CD64-positive cell line U937 towards Granzyme B was tested by transfection with a plasmid allowing cytosolic expression of Granzyme B. As the target cells were successfully killed by Granzyme B, and thus the effectivity of the transfection-based test system for toxin sensitivity was proven, the Granzyme B-based CD64-targeting immunotoxin (E)Gb-H22(scFv) was constructed. Different expression systems (E. coli, HEK293T cells, CHO cells, FlpIn 293 cells, apoplast/chloroplasts/ER of N. tabacum) were evaluated, among which HEK293T cells were finally used for production. Different cell culture formats including microcarrier-based fermentation were established. Downstream processing was optimised in terms of handling convenience, required time and in particular purity and integrity of the end product. In order to clarify the role of a free NH2 terminus for a Granzyme B-based immunotoxin, enterokinase-cleaved Gb-H22(scFv) was compared with uncleaved EGb-H22(scFv) in a proteolysis assay with a synthetic Granzyme B substrate. Only the activated Gb-H22(scFv) showed the same proteolytic activity as free Granzyme B, whereas the uncleaved EGb-H22(scFv) failed to cleave the synthetic substrate. Therefore, further experiments were focused on Gb-H22(scFv), although EGb-H22(scFv) was also cytotoxic, albeit with about 10-fold lower IC50 values compared to Gb-H22(scFv). For all cell-based analyses, the stimulation in particular of U937 target cells with IFN-gamma was optimised with regard to concentration, physiological state of the cells (time point for stimulation after passaging), and time frame for cytotoxicity assays. Concerning its biological activity, Gb-H22(scFv) was shown to bind to CD64+ U937 cells, but not to CD64- Raji cells, and exhibited potent specific cytotoxicity towards U937 cells with IC50 values in the range of 1.7 to 17 nM. These values are lower than the majority of published IC50 values of other Granzyme B-based immunotoxins and thus demonstrate high cytotoxic efficacy. The induction of apoptosis was successfully detected via phosphatidylserine exposure, caspase-3 activation or mitochondrial depolarisation in U937 cells, primary peripheral blood mononuclear cells of a healthy donor, and primary acute myeloid leukaemia blasts. Based on these in vitro studies, optimised structural variants of Gb-H22(scFv) were constructed, expressed and characterised in terms of binding activity. In conclusion, the present work laid the basis for subsequent in vivo experiments in the characterisation of Gb-H22(scFv) and its derivatives.

Die Akute Myeloide Leukämie (AML) ist eine heterogene Gruppe von Krebserkrankungen des blutbildenden Systems. Ihre Subtypen M4 und M5 sind durch eine erhöhte Expression von CD64 (Fc-gamma-Rezeptor I), dem Hochaffinitäts-Rezeptor für IgG, gekennzeichnet. Während konventionelle Therapieformen wie Chemo- und Radiotherapie unspezifisch Zellen mit hoher Proliferationsrate angreifen und somit unerwünschte Nebeneffekte durch die Elimination gesunder Zellen hervorrufen, können Immuntoxine spezifisch die malignen Zellen binden und abtöten. Die Verwendung humaner Proteinkomponenten, wie beispielsweise der humanen Serinprotease Granzyme B als Toxin, trägt hierbei wesentlich zur Sicherheit und Nebenwirkungsfreiheit eines Immuntoxins bei. In dieser Arbeit wurde das erste humanisierte Granzyme B-basierte Immuntoxin gegen CD64-positive Zellen entwickelt, evaluiert und modifiziert. Als Vorab-Studie hierzu wurde die Sensitivität der AML-verwandten CD64-positiven Zelllinie U937 gegenüber Granzyme B getestet, indem die Zellen mit einem Plasmid transfiziert wurden, welches die cytosolische Expression von Granzyme B erlaubt. Da die Zellen erfolgreich durch Granzyme B abgetötet wurden und somit auch die Effektivität des transfektionsbasierten Testsystems zur Toxinsensitivität gezeigt wurde, wurde das Granzyme B-basierte anti-CD64-Immuntoxin (E)Gb-H22(scFv) konstruiert. Verschiedene Expressionssysteme (E. coli, HEK293T-Zellen, CHO-Zellen, FlpIn-293-Zellen, Apoplast/Chloroplasten/ER von N. tabacum) wurden evaluiert, von denen HEK293T-Zellen schließlich zur Produktion genutzt wurden. Verschiedene Zellkulturformate einschließlich microcarrierbasierter Fermentation wurden etabliert. Das Downstream Processing wurde in Bezug auf die Einfachheit des Handlings, die benötigte Zeit und insbesondere die Reinheit und Integrität des Endprodukts optimiert. Zur Klärung der Rolle eines freien N-Terminus für ein Granzyme B-basiertes Immuntoxin wurde enterokinaseverdautes Gb-H22(scFv) mit unverdautem EGb-H22(scFv) in einem Proteolyseassay mit einem synthetischen Granzyme B-Substrat verglichen. Nur aktiviertes Gb-H22(scFv) zeigte dieselbe proteolytische Aktivität wie freies Granzyme B, wohingegen das unverdaute EGb-H22(scFv) nicht in der Lage war, das synthetische Substrat zu schneiden. Daher wurden die folgenden Experimente auf Gb-H22(scFv) fokussiert, obwohl EGb-H22(scFv) ebenfalls zytotoxische Aktivität zeigte, wenn auch mit etwa zehnfach niedrigeren IC50-Werten als Gb-H22(scFv). Für alle zellbasierten Analysen wurde die Stimulation insbesondere von U937-Zellen mit IFN-gamma im Hinblick auf die Konzentration, den physiologischen Zustand der Zellen (Zeitpunkt der Stimulation nach dem Passagieren) und das Zeitfenster für Zytotoxizitäts-Assays optimiert. Hinsichtlich seiner biologischen Aktivität konnte gezeigt werden, dass Gb-H22(scFv) spezifisch an CD64-positive U937-Zellen, jedoch nicht an CD64-negative Raji-Zellen bindet, und eine potente spezifische Zytotoxizität gegen U937-Zellen mit IC50-Werten zwischen 1,7 und 17 nM aufweist. Diese Werte liegen niedriger als die Majorität der veröffentlichen IC50-Werte anderer Granzyme B-basierter Immuntoxine und demonstrieren somit eine hohe zytotoxische Effizienz. Die Induktion der Apoptose wurde über die Phosphatidylserin-Exposition, die Caspase-3-Aktivierung oder die mitochondriale Depolarisation erfolgreich nachgewiesen, dies sowohl in U937-Zellen als auch in primären mononukleären Zellen des peripheren Blutes eines gesunden Spenders sowie eines Spenders mit Akuter Myeloider Leukämie. Basierend auf diesen In-vitro-Studien wurden optimierte strukturelle Varianten von Gb-H22(scFv) konstruiert, exprimiert und hinsichtlich ihrer Bindeaktivität charakterisiert. In der Zusammenfassung legt diese Arbeit den Grundstein für spätere In-vivo-Experimente zur Charakterisierung von Gb-H22(scFv) und seinen Derivaten.
